Southend West Central Primary Care Network (PCN) has been named PCN of the Year at the Integrated Health Primary Care Impact Awards.
The awards celebrate innovation, new ways of working and good practice across primary care in the NHS and beyond through categories which explore teams making a difference through patient access, digital solutions and more.
Our nomination for the award highlighted the work the team has done at community outreach events, care home ward rounds and the excellent progress made by the PACT (PCN Aligned Community Team) model, which supports patients on discharge from hospital by assessing and ensuring referrals as required to assist with recovery and to avoid re-admissions.
The winners were announced on Thursday 14 December, with the judges saying, “Southend West Central PCN took the top spot in this category, with their commitment to demonstrating the change and brilliance we can make together.”
